ldif_open_mem() is the fmemopen(3) equivalent of ldif_open() which opens
an ldif steam from memory, rather than from a file.

Doc has been updated to note the default was actually 78.
The off-by-two error is fixed. Note that wrap=1 will still
output 2 columns, otherwise it can't output anything besides
the continuation character.

Was rejecting "-\r\n" in input LDIF

Also removes a bit unnecessary code.

ldif_read_record() doesn't distinguish errors from EOF, this is kind
of futile.
