Add new entry_dup_bv that dups an entry in a single malloc. Leave it
unused for now; faster but consumes more heap.

the entry to get nvals first, so the size can be computed.

Note: this breaks LDAP_COMP_MATCH and SLAP_ZONE_MALLOC. But they
were probably broken already anyway.
