Initial check of connection states. Have only implemented 

SLAP_C_ACTIVE vs SLAP_C_INACTIVE.  Need to implement BINDING and CLOSING.
Added fields tracking pending (on bind) ops.  Could also be used to
implement per-connection thread limits.
