If an add or modify op referenced a previously unused attrtype,
the mi_ads/mi_numads would be incremented to include it. If the
op fails to commit, these additions must also be reverted.

wsize was being truncated to 32bits on Windows.
Only try to write 1GB at a time on Windows64;
larger writes fail with ERROR_WORKING_SET_QUOTA.
